  3. Return address which must match the one registered to your Stake.us account.
  4. E-mail address that is the same as on your Stake.us account.
  5. Clearly write the word “CODE" followed by your unique code.

Now you’re ready to write the following statement:

“I wish to receive Stake Cash to participate in the sweepstakes promotions offered by Stake Sweepstakes. By submitting this request, I hereby declare that I have read, understood, and agree to be bound by Stake’s Terms and Conditions."
