The paper should be unfolded, with text written only on one side.

  1. Full name as shown on your government-issued ID.
  2. username.
  3. Return address which must match the one registered to your account.
  4. E-mail address that is the same as on your account.
  5. Clearly write the word “CODE" followed by your unique code.

Now you’re ready to write the following statement:
